A clinically integrated network helps physicians align with the hospital to coordinate care across health care providers, focus on quality and performance, and prepare for new, incentive-based compensation programs in addition to the base compensation they already receive. The network will develop new payment systems and methods that focus on achieving quality, efficiency, cost-management measures and enhancing value for patients.

Clinical integration is an effort among physicians, often in collaboration with a hospital or health system, to develop active and ongoing clinical initiatives focused on delivering quality, performance, efficiency and value to the patient.
What types of health data will Health Leaders Network review

Your doctor will look at data that can help improve the outcome of treatments for specific conditions, as well as the overall health of the patients for which your doctor cares. For example, for patients with diabetes or high blood pressure, we will measure how your doctor uses the most up-to-date and proven practices for helping patients manage their conditions. We will also look at how many patients overall receive preventive care such as flu shots, mammograms, colonoscopies or advice on healthy living. When gaps are identified, for example if a patient is overdue for a mammogram, your doctor will be notified so he or she can contact you for follow up.

The current clinical programs and network initiatives improve quality outcomes and control costs to manage patients at highest risk for poor outcomes and high cost.

The network will be able to improve care transitions and enhance access for patients by developing evidence-based medicine protocols, data and referral processes, and access standards across the continuum of care. These improvements will enable primary care physicians to manage the care of their patients more effectively and allow specialists to focus their efforts on those patients who need specialized care.
